Clinic Cancellation and Inbasket Coverage Form aka A323 The eA323 is the form used by the Residency Office the Medical Staff Office to cancel clinics (and preceptors for your clinics) and to arrange for inbox coverage. The county requires that you give 35-day advance notice in order to prevent providers from canceling clinics on short notice, which can be very frustrating for patients that have been waiting for weeks to be seen. Please make sure to fill the entire duration of your time off in the "From and Through DATES:" section (see screen shot below). This is how the office knows which days to schedule inbox coverage for you.
Once the form is completed, email it to Patricia Pharris-Spruell [pharis-spruell at ccfamilymed.com] along with approval from Scheduler and Rotation Supervisor.
